In 2009, a group of experienced divers experienced a terrible event. It was stated that a malfunction in one of the divers’ main regulators caused the gas supply to be cut off suddenly and completely. In the chaos that followed, the diver was unable to reach its secondary regulator due to equipment confusion, possibly due to inadequate configuration or improper maintenance. Tragically, the stranded diver lost consciousness during the ascent and drowned before the crew could reach the surface.
